Paycheck Stubs (last 6 months; up to date/day of filing Bankruptcy)
*Typical paycheck; please list average hours and overtime worked
* Children’s paystubs, who live in the home (last 6 months)
*If married, include spouse’s paycheck stubs (last 6 months)
Mortgage Payoff Statement
*Balance of mortgage owed; creditor who holds balance
(mortgage amount remaining) (who mortgage is with)
*Home Equity Loans – Balance and creditor
Car Loan
*Creditor and amount owed (who loan is with and amount left on vehicle)
Photocopy of Car Titles
Photocopies of last 4 years Income Taxes filed
*Federal and State Income taxes – including 2009
All “Collection” Letters
*Please have original bills
All Bills – *Minus Utility Bills
*Including medical bills and loans (if money owed)
*If you submit Credit Report, circle accounts that currently apply
- Credit Reports do not contain Account Numbers
*Account Numbers and Creditor Addresses are mandatory
Names and Addresses of Co-debtors
*Anyone who is also on a Loan with you
Foreclosures
*Name of attorney and date or deficiency (amt. of money) owed
Repossessions
*Creditor (who loan is with) and deficiency (amt. of money) owed
Summons, Complaints, Judgments, and Lawsuits
Closed Bank Account Information
*Bank, account #, amount left at closing, and reason for closing
Transfers
*Person to whom sold, amount sold for, and condition of item (good, poor, fair)
